The Churchill Physical Health Center, S.C. was established in 1956 by Dr. Churchill. Originally located in Lockport, the office made its first move to the lower level of Dr. Churchill's home and then relocated to its present location in 1974. In 1992 Dr. Churchill retired to Florida and placed the practice in the capable hands of his stepson, Dr. Ted Cook.

Dr. Cook attended Northern Illinois Unversity and the College of St. Francis majoring in business. He then joined the family adhesives business and worked there for five years before returing to school to pusue his goal of becoming a chiropractor. While in chiropractic college, Dr. Cook was selected to be an assistant instructor in physical diagnosis and spinal manipulation. He received a Bachelor of Science degree in 1988 and a Doctorate in Chiropractic in 1991, from Logan Chiropractic College in St. Louis, Missouri, graduating cum laude.

Soon after graduation Dr. Cook was asked to be the strength coach for the Joliet Jets swim team for two years.

Dr. Cook continued his post graduate studies and became certified in 1998 to perform Manipulation Under Anestheria on complicated spinal cases. He is currently working with the Water Tower Surgery Center.

In addition to his extensive post graduate course of studies, Dr. Cook is certified to do Impairment Rating and the Cox Flexion/Distration technique.

Dr. Cook is currently the owner of Churchill Physical Health Center S.C. with his wife, Dr. Rhonda Cook.

Dr. Cynthia Kadela graduated from National College of Chiropractic in 1981. She was the first female chiroprator in Will County and has been in practice at the Churchill Clinic for 23 years. Her post graduate education has included seminars at the renowned Gonstead Clinic.

Dr. Cook's wife, Dr. Rhonda (Bryden) Cook, graduated from Logan College in 1990 and joined the Clinic in 1992.

Sarah Evans is a nationally certified and state licensed clinical massage therapist.  For the past year, she has worked in physical therapy with chronic pain patients before starting her own business with partner and former teacher, Kelly Fowler.  Sarah graduated as valedictorian with high honors in Anatomy, Physiology, Clinical Massage Technique and Orthopedic Testing from the Soma Institute where she has since been teaching.  As a student, she represented the Soma Institute at the Chicago Marathon and Northwestern Memorial Hospital, Oncology Department.  Recently, Sarah was trained in the ancient Hawaiian massage tradition, Lomi Lomi.

Kelly Fowler is a graduate of The Soma Institute in Chicago.  She has six years experience as a clinical or orthopedic massage therapist.  During her training she has developed extensive knowledge of massage techniques, musculoskeletal posture and gait analysis, and treatment planning.  She has applied her experience to several different massage arenas:  Sports Massage at the Chicago Marathon for the past 4 years, orthopedic massage in several clinics in the Chicagoland area, and as an instructor of Anatomy and Clinical Massage Technique at The Soma Institute.  In addition to her clinical knowledge she has also studied a variety of massage styles that complement her orthopedic work.  Pregnancy massage, Ortho-Bionomy, and Reiki, Shiatsu and Nuat Thai (all forms of Asian bodywork) are just a few examples.


Tarek El-Shikh, MSPT:
Tarek is a 1987 graduate of the Faculty of Medicine, Cairo-Egypt. He holds a master's degree in physical therapy and rehabilitation.

Tarek specializes in acute and chronic orthopedic, soft-tissue, and sports-related injuries. He has extensive experience in treating low back injuries, including SI joint dysfunction and improper pelvic alignment. He also has an active interest in working with high school, college, and professional athletes. He chose to be a physical therapist because it is a challenging, ever-changing profession where he can work with a wide variety of individuals and help them return to their normal, everyday functions. Tarek is a member of the Orthopedic Section of the American Physical Therapy Association and the Private Practice Section. He is specializing in manual therapy and holds a certification program from the McKenzie Institute in Mechanical Spine evaluation and treatment.
